Group Records Change form

Here is the form to submit to your District 24 Al-Anon Family Groups (AFG) to register a new group in Monterey or San Benito Counties, or to make changes in information about an existing group, such as new name, time, day, address; new mailing address for the group; new group representative, etc. This form helps us maintain good records of our groups. Follow the directions on the bottom of the form.  Mail the completed form to the Group Records Coordinator at the Northern California World Service Area (NCWSA) and a copy to District 24 Secretary, PO Box 51901, Pacific Grove, CA 93950.  You can also send the completed form to the email address for the District Representative. This helps to keep our groups in touch with other groups. The NCWSA newsletter Twelve Stepper will be sent to the Group Representative. The Twelve Stepper contains much information our Group Representatives need to be informed voters at Area Assemblies, as well as information about how and what other districts in Northern California are doing.

Expense Reimbursement Form

From time to time, members in service may spend their own money for District 24 business: buying supplies for an event, making copies, or paying for travel on District business. To request reimbursement from the District 24 Treasurer, please submit this form, along with your receipts.

Advance for expenses

When members in service need to pay for something for District 24 business, but do not wish to use their own money and receive reimbursement afterwards, they can submit this form to request an advance of money to cover the expense.
